I’m applying for disability. What do I bring to My appointment?
If we are going to be filing a new application with you, there are a few things that will make the process run smoothly. Please bring the following:
The login and password for your myssa account (more details HERE)
A list of doctors/hospitals/clinics that have treated you since you became disabled. If you have them, bring the login and password for your online portals (myochsner, LCMC health, etc) so that we can pull the information that we need directly from their system.
A list of current prescription medications, including when you started taking it, which doctor prescribes it, and what you take it for
A list of jobs that you have worked in the past 15 years (to the best of your memory) or a resume covering that same time period
With the information above, we should be able to quickly and efficiently submit your application.
